Cardiovascular Disease is the second largest cause of overall disease burden and second most common cause of death after cancer in Scotland. Many people do not know that they are living with CVD risk factors and a key objective of the Scottish Government Cardiovascular Risk Factors Programme is to optimise early intervention and care to therefore reduce patients risk of developing CVD.
Patients are eligible for checks if they meet the following conditions:
Are aged 35 – 60 and have not received a full bundle of CVD checks in the last 3 years :-
Blood Pressure check, Blood tests for Cholesterol, Glucose, HbA1c, Smoking / Alcohol status / Height / Weight / Body Mass Index (BMI)
The practice will be contacting eligible patients by text message. If you wish to take part, please text back with the reply ‘Yes’ and a member of our admin team will be in touch to offer you an appointment.
Your check up will involve 3 appointments:-
1st – Staff will take your blood pressure, height, weight, body mass index, smoking & alcohol status and blood test.
2nd – Follow up appointment with the GP to discuss your results
3rd – 6 months later, you will have your final follow up appointment with a GP, if required.
